Keep Your Certification Current: Iowa DBE Renewal Process Guidelines

This is a friendly reminder regarding the annual DBE certification renewal process. The annual eligibility review, mandated by the Iowa Department of Transportation (DOT), is a crucial step to maintain your certification status. The anniversary month of your DBE certification prompts you to complete your renewal.

To comply, fill out the form entitled “Affidavit of Continued DBE Certification Eligibility” found at the following link: https://iowadot.seamlessdocs.com/f/AffidavitofContinuedDVECertificationEligibility

Carefully review the certification requirements, fill out all blank fields and boxes, and submit all requested documentation accurately and completely to ensure continued eligibility for certification.

Specifically, make sure on the Affidavit of Continued DBE Certification Eligibility

1) The Report Gross Receipts for the Attached Current Tax Return box is completed and matches the gross receipts from the tax return.

2) The Ethnic Group and Sex is listed for all owners.

3) The submitted tax return includes the eFile Signature Authorization Form or if the tax return was submitted by physical mail, then a signed and dated copy (as it was submitted to the IRS) is included with your submittal.
